How did the screw migrate from the BACK of the ice machine, into the cup of iced coffee?

If it was from the outside/back of the machine, that would seem impossible.

If it was from the inside.....I'd like to know about the design brilliance involved; and how they would prevent such a thing from happening again.

That looks like something from the outside of the machine holding the sides on.
 
Yes, looks like a thumb screw.

I am by no means a legal expert but if that was a thumb screw, someone was either very careless or this was very deliberate. Those sorts of things are on the outside of the machine and don't just end up in food on their own.
 
It's probably from a panel that is frequently opened and closed for cleaning. Maybe the staff didn't properly tighten the screw and it fell off, or they had set it aside and it fell into the drink. Just speculating.
 
Happened to notice yesterday that the one on Lakeshore rd just east of Southdown in Clarkson had closed. Place was overgrown. Not sure if it was just a lease expiry or what but it looked awful.

There's a lot of history at that location. It used to be a variety of gas stations dating back to pre-war times in Clarkson Village. I believe around the 60's or 70's the existing building was constructed and housed the local Royal Bank until about 2000-01 when the current RBC branch was built across the street. Tim's came in a few years later and occupied the space until 2020. COVID absolutely decimated their business as it was walk in only without the advantage of a drive-thru.

There's a high concentration of Tim's in that area with one on Lakeshore-Southdown south of the GO station, a mini-kiosk with a drive-thru at the Canadian Tire gas station north of the GO station, another walk-in location beside Rexall on Southdown further north and then another location on South Service Rd at the QEW-Mississauga Rd interchange. To be frank, there's too many Tim's in the area and this location had the disadvantage of being less accessible than the others, especially without a drive-thru.

Funny, that was one of the Tim's with the Cold Stone co-locations too (remember that?) - the store had lots of seating and was a useful rest stop when cycling through.
