The best vanilla milkshake I've ever had in Toronto?
Blackjack Grille, Keele St just south of Dundas St West. Made by the owner's (Italian) mother. Sickeningly good, and quite cheap. (It's the only reason I go there.)
Please. Give respect where respect is due. I was prescribed milkshakes when I was a child because I was under-sized. I used to make shakes so large I had to stagger outside into our yard to let the sun shine on my tummy until I regained conciousness.
So I will tell you where the best shake is.

On a related point, Vanity Fair's article on Stern's 15 CPW described it as "the most successful apartment building in history". At some point this sort of universal praise is going to become over the top, although we're not there yet.
I enjoy Fran's, but lately I've been going into Sandwiched (Church south of Carlton) to satisfy my cravings. They have some creative shakes, including one awesome one that has peanut butter and honey in it
Me, whenever I get my 7-11 Slurpee fix, I like to add some flavour dashes of hazelnut or cinnamon or whatever from the coffee section. Don't know if others do so
